INFORMED CONSENT FORM

1. This section provides an explanation of the study in which you will be participating:

In this study, we are examining various cognitive processes involved in perception and thinking. During the study, you will read words or see pictures and/or think about words and pictures that are no longer present. You will also be asked to engage in a short writing task and complete a short questionnaire. The risks associated with participating in this study are no greater than those you would encounter in everyday life. You will be informed further about the purpose of this study at the conclusion of your participation.

2. This section describes your rights as a research participant:

In return for your participation, you will receive $15. Your participation is completely voluntary and you may change your mind and terminate the study at any time without loss of benefits.

Your participation in this research is confidential. All data collected will be kept strictly confidential, and only code numbers will be used to identify your responses. The data may be included in a public data repository when any manuscript relating to this study is submitted for publication and after publication. However, the data will be coded so as not to identify you by name.

3. This section indicates that you are giving your informed consent to participate in research.

I am at least 18 years of age. I understand the information given to me, and I have received answers to my questions about the research procedure. I understand and agree to the conditions of this study as described. I understand that I will receive $15, and that I am entitled to no other compensation. I understand that my participation is voluntary and I can withdraw from this study at any time by notifying the person in charge.
